ORDER FORM
Chick-fil-A will be delivering to Centre Street Church for lunch time only. Meals will be delivered at 12:15pm for pickup at the SW Playtrium on the main floor of Centre Street Church. All orders must be sent by Friday May 1 2026 or the order will not be filled.
Credit card payments will be processed the morning of delivery, Saturday May 9 2026.
